While not a default setting found in all surveys, the z-value can be enabled by inputing esriFieldTypePointZ into the bind::esri:fieldType field in the Surveys tab. You may want to consider using an external GNSS receiver in addition to the mobile collection device (smartphone or tablet) as this improves measurement consistency. If you need to capture the point values in separate fields, then you will want to create an additional question in the data collection form. This additional question would be hidden, use a pulldata function in the calculation field, and be set as esriFieldTypeDouble in the bind::esri:fieldType field.
